Client Services

Outreach and in-center services are available for older adults who are in need of assistance in finding the right resource to help with their needs. Client services can connect individuals with in-center services and can provide information and referrals to community based resources to meet specific needs such as housing, nutrition, transportation, etc.  When the family needs assistance our Client Services Coordinator can arrange to meet with family members about issues such as health and well being, safety, care giving, housing, and respite services.  

Home Visits

The Client Services Coordinator is available to visit the home to help coordinate services, and assist families in resolving problems and maximizing independence.
